Passages

Full time case management is provided for families in the Family Promise/IHN program as part of a six-agency collaboration funded by The US Department of Housing and Urban Development. Professional case managers assist families with the following issues: housing, employment, job training, education, childcare, counseling, legal issues and many other needs. Case managers work with the Passages partner agencies, as well as the community at large, to ensure that each family successfully moves toward self-sufficiency.

Features of Passages:

  • Long-term, comprehensive case management lasting up to 18 months
  • Through weekly meetings, professional case managers partner with clients in establishing realistic goals that guide each family toward self-sufficiency
  • Case managers provide support services to each family for 6—15 months after they transition out of a shelter program
  • Provides subsidized childcare for up to one year
  • Provides mental health, substance abuse, and family counseling services
  • Provides direct financial assistance
  • Provides transitional housing opportunities
  • Provides Life Skills seminars
